Mirta Millares (PharmD, FCSHP, FASHP) has over 28 years experience in drug information practice at Kaiser Permanente (KP). Formerly, she was clinical faculty for the University of Southern California School of Pharmacy with a full-time practice at the USC Kenneth Norris Jr Cancer Center. While at KP, she held an adjunct faculty appointment at USC School of Pharmacy.
As Director at KP Drug Information Services and Outcomes Research in California, she led a department of over 35 staff with responsibilities for management of Commercial Formularies for The Permanente Medical Group in N Cal and the Southern Cal Permanente Medical Group, as well as the KP National Medicare Part D Formulary. Her team developed a new drugs forecast report semiannually to support rate-setting and operational budget cycles. She was responsible for the coordination of drug recalls and shortages, the development and maintenance of drug information for the EMR in S Cal, and communication of REMS and medication safety alerts. She provided oversight for the Pharmacy Outcomes Research Group which completed a variety of studies resulting in publication.
Mirta served as Director of the KP PGY2 Drug Information Residency Program for 25 years and provided APPEs for multiple schools of pharmacy, and drug information experiences for KP PGY1 and PGY2 residents.
She is the Editor of the textbook, Applied Drug Information: Strategies for Information Management. She is on the Board of Directors of the California Society of Health-System Pharmacists (CSHP) and was the recipient of the CSHP Distinguished Service Award (2008) and SCSHP Pharmacist of the Year Award (2003).
Mirta Millares earned her Doctor of Pharmacy degree from the University of California School of Pharmacy in San Francisco (UCSF). She completed a PGY1 Clinical Pharmacy Residency and a PGY2 Residency in Drug Information also at UCSF.
